The undercoded one
Established patient on warfarin with an unstable INR, dose adjusted, repeat testing scheduled, dietary and interaction counseling documented, bleeding risk assessed. Warfarin management with active instability is the textbook example of drug therapy requiring intensive monitoring for toxicity. Combined with a chronic illness that is not controlled, this supports 99215. It is very frequently coded 99214 out of caution.
Documentation that holds
- Name the severity explicitly. “Severe exacerbation” and “poorly controlled” carry weight that “flare” does not.
- Where risk is your high element, write the risk decision as a decision: what you weighed, what you chose, why. Intensive monitoring means monitoring for toxicity, so say what you are monitoring and why.
- Where hospitalization was considered and rejected, document that it was considered. A note that omits the deliberation reads as a routine visit.
- If coding on time, record the actual minutes and their composition. Forty to fifty-four minutes is a narrow band and cloned time statements are a recognized audit flag.
