What is the 99214 CPT code?
99214 is the level 4 code in the established patient office and outpatient series, 99211 through 99215. Under the AMA’s 2021 E/M framework, still the governing structure in 2026, you qualify by either of two independent routes.
- Medical decision making: moderate complexity, meaning two of the three MDM elements reach the moderate level.
- Total time: 30 to 39 minutes of the physician’s or qualified health professional’s own time on the calendar date of the encounter, including record review, documentation, ordering, and care coordination, not only face-to-face minutes.

What does 99214 pay in 2026?
Two things changed for 2026 and both matter. CMS finalized separate conversion factors, $33.5675 for qualifying APM participants and $33.4009 for everyone else, and it reallocated indirect practice expense away from facility settings. The result is that office-based visit payment rose while the same code in a hospital outpatient department fell.

The 99214 office rate rose about 8.3% from $125.18 in 2025. The facility rate fell close to 10% from $93.80. That divergence is the most consequential fee schedule story of the year for hospital-employed physicians, and it is entirely driven by the practice expense component. Add-on code G2211 remains available for visits where you serve as the continuing focal point for a patient’s ongoing care, adding roughly $16 to $19. For 2026 it also became reportable with home and residence E/M codes. One restriction catches practices repeatedly: G2211 cannot be billed alongside modifier 25 unless the paired procedure is a Part B preventive service, an immunization, or an annual wellness visit.

What counts as moderate MDM?
MDM has three elements. You need two of the three at the moderate level.
- Problems addressed: one or more chronic illnesses with exacerbation or progression, two or more stable chronic illnesses, an undiagnosed new problem with uncertain prognosis, or an acute illness with systemic symptoms.
- Data reviewed and analyzed: moderate means categories such as ordering and interpreting tests combined with an independent historian, independent interpretation of a test another provider performed, or discussion of management with an external physician.
- Risk of complications: moderate, for which prescription drug management is the everyday anchor. Decisions about elective major surgery, or a diagnosis affected by social determinants, also qualify.
A diabetic with hypertension whose medications you adjust after reviewing labs is the textbook 99214: chronic illness with progression, moderate data, prescription management. A stable patient on unchanged medication with normal results is usually 99213. The full contrast is in 99213 versus 99214.
Documentation requirements that survive an audit
- Name each problem and state its status: stable, worsening, improving, or new.
- Show the data work explicitly. What you ordered, what you reviewed, whose records you read, who you spoke to.
- Make risk visible. Name the prescription decision, the dose change, or the monitoring choice. Risk that exists only in the auditor’s imagination does not count.
- If coding on time, record the actual total minutes and what the time comprised. “Total time 34 minutes, including review of outside cardiology records and after-visit care coordination” is defensible. “Time spent 30 to 39 minutes” as a template macro is a red flag, because cloned time statements appear identically across hundreds of charts.
- Write the assessment and plan so the decision is legible. Auditors and payer algorithms both level on decisions, not on note length.
Common 99214 denial and downcoding traps in 2026
The dominant risk to 99214 is no longer outright denial. It is silent reduction.
Cigna’s R49 Evaluation and Management Coding Accuracy policy, effective 1 October 2025, permits automatic downcoding of level 4 and 5 office visit codes, specifically 99204 and 99205, 99214 and 99215, and 99244 and 99245, by one level where the submitted information does not appear to support the complexity billed. Aetna operates a comparable clinical claims review program expanded through 2025. Cigna has said it expected around 99% of in-network clinicians to be unaffected at launch, which is reassuring in aggregate and irrelevant if you are in the other 1%.
Three operational consequences follow, and they change how a billing team must work.
- There is no denial to find. The claim pays, just at 99213. Unless somebody reconciles the CPT submitted against the CPT paid on the remittance, the loss is invisible. Practices have discovered five-figure quarterly leaks only after manually combing remits.
- Modifiers do not prevent it. Appending modifier 25 or 59 has no bearing on a downcoding edit.
- Recovery requires an actual appeal. Cigna will reimburse at the original level when records substantiating the higher level are submitted, but the practice has to submit them, and the process has been criticised for relying on fax.
Regulators have started responding. The Maryland Insurance Administration fined Cigna $80,000 in March 2026 and ordered it to cease automatic downcoding, California’s Department of Managed Health Care paused comparable policies pending review, and Indiana enacted restrictions in 2026. Appeal rights in this area are therefore worth more now than they were twelve months ago.
The older traps have not gone anywhere: cloned notes, an MDM section that restates the problem list without recording any decision, missing time documentation on time-based claims, and 99214 billed alongside a minor procedure without a defensible modifier 25.
